How Membership Works

1. We’ll take the time to understand you. We'll examine your medical history, conduct a series of lab tests, and work together to establish your health goals.
2. We’ll go over your test results together. You’ll get a detailed analysis, and we’ll collaborate to develop a personalized plan designed to help you achieve your health goals.
3. We’ll arrange a 6-week follow-up appointment to re-evaluate your bloodwork and track your progress. This gives us the opportunity to refine your plan and make any needed adjustments.
4. You'll have direct access to your Integrative Health Team throughout the entire process. Feel free to ask questions and communicate with your provider via email, text, or through our patient portal.
5. We invite you to compliment your Integrative Health Program with our aesthetic services. Membership is not required for aesthetic services, however, we have two additional membership options to maximize your savings.

Integrative Health Menu

Visits:

• Initial Follow Up - $200
• Subsequent Follow-Ups - $150
• No Show Fee: A $150 fee will be charged for appointments canceled within 24 hours or if the patient does not show up.

Bio Identical Hormone Pellet:

• Male: $600 – $750 | Lasts 4 – 6 months
• Female: $400 | Lasts 3 – 4 months

Labs:

• Baseline Labs – Self Pay: $200 (additional labs may be required)
• Post Panel Labs - $125

Inbody:

• Unlimited scans included with membership
• Single Session: $35
• Set of 2 Sessions: $60
